Metastatic Renal Cell Carcinoma

Renal Cell Carcinoma (RCC)
, also called Renal Adenocarcinoma or
Hypernephroma, is the development of cancerous changes in the cells of the renal
tubules.
RCC is the most common kidney cancer, affecting approximately 40,000
people in the U.S. every year.

The symptoms of
RCC include blood in the urine, abnormal urine color, flank or
back pain, a lump or mass in the kidney area, abdominal pain or swelling, weight
loss, testicle enlargement, fatigue, fever and anemia.
RCC has the potential to
invade organs near the kidney such as the liver, colon, or pancreas.

Metastatic Renal Cell Carcinoma is the term applied to advanced kidney
carcinoma where the disease has spread to tissue and organs distant from the
original site.  About a quarter to a third of patients have metastases at diagnosis.
Interestingly, PROLEUKIN IL-2 is the only therapy FDA-approved for the treatment
of metastatic renal cell carcinoma.

Clinical History/Findings

The patient is a 65 year-old white female with renal, pulmonary, and liver masses.
The patient also has a history of breast CA.

Specimen Type

An ultrasound guided fine needle aspiration biopsy (FNAB) of the liver was
performed.

Cytologic Findings

Cytologic Dx:    Metastatic Renal Cell Carcinoma

IHC Results:

Commentary:  These results are most consistent with metastatic renal cell carcinoma.
Other primaries, though much less likely are still possible

Slide 1: Clear Cell RCC.  Diff-Quik.
Numerous fine vacuoles are present
giving a clear appearance outined
against the darkly stained
cytoplasm.
Slide 2:  Clear Cell RCC.  Pap Stain.
The cytoplasm is abundant, wispy,
delicate, and pale with optically clear
areas. The nuclei are enlarged with fine
pale chromatin,  prominent nucleoli,
and thick, irregular nuclear borders.
